SPECIFIC CLINICAL DUTIES. The LPN shall provide services within the scope of their competencies and the applicable duties. Additionally, the LPN shall perform the following:

* Provide care coordination using specific criteria and guidelines as mandated by DOD, BUMED, and TRICARE directives, coordinating with the local MHP clinic, utilization and Referral Management policies, and obtains additional clinical data as required.

* Consult, coordinate, and collaborate with MHP team members, referral management team members, specialty care clinics, outside care professionals, agencies, organizations and other ancillary support systems to assist in the care coordination of MHP patients.

* Identifies patients in need of care coordination within the MHP teams, through screening patients with a pattern of high risk, unstable and resource intensive utilization of Military Treatment Facility (MTF) or network services such as chronic pain patients.

* Responsible for coordination of outpatient care services of patients belonging to the MHP clinic area of responsibility that are discharged from other facilities.

* Follows-up with MHP team members regarding review of patient charts, medical records, and other documents utilized in the treatment and care of MHP patients in need of care coordination.

* Participate in an on-site designated Extension for Community Healthcare Outcomes (ECHO) tele-mentoring training program.

* Participate in tele-mentoring, tele-medicine, and tele-consults.

* Assist the Physician with the placement of radiology equipment (C-arm) for medical procedures that require imaging guidance.

GENERAL DUTIES: LPNs shall perform a full range of duties consistent with their labor category, education, training, experience, clinical privileges (as applicable), and assigned position. General duties that apply to all personnel or across certain labor categories are given below. Duties that apply to all personnel are as follows:

* Maintain a level of productivity comparable with that of other individuals performing similar services.

* Participate in peer review and performance improvement activities.

* Practice aseptic techniques as necessary. Comply with infection control guidelines to include the proper handling, storage, and disposal of infectious wastes, and the use of universal precautions to prevent the spread of infection.

* Function with an awareness and application of safety procedures.

* Perform efficiently in emergency patient situations following established protocols, remaining calm, informing appropriate persons, and documenting events. Anticipate potential problems/emergencies and make appropriate interventions. Notify supervisor, director, or other designated person regarding problems that the HCW is unable to manage.

* Apply an awareness of legal issues in all aspects of patient care and strive to manage situations in a reduced risk manner.

* Participate in the implementation of the Family Advocacy Program as directed. Participation shall include, but not be limited to, appropriate medical examination, documentation, and reporting.

* Exercise awareness and sensitivity to patient/significant others' rights, as identified within the MTF.

* Maintain an awareness of responsibility and accountability for own professional practice.

* Participate in continuing education to meet own professional growth.

* Attend and/or comply with all annual training classes required by the Command, to include but not limited to online annual training provided by the MTF: disaster training, infection control, Sexual Harassment, Bloodborne Pathogens, Fire and Safety, Chemical, Biological, Radiological, Nuclear and Explosives (CBRNE), and all other required training.

* Actively participate in the commands Performance Improvement Program. Participate in meetings to review and evaluate the care provided to patients, identify opportunities to improve the care delivered, and recommend corrective action when problems exist.

* Participate in the provision of in-service training to clinic staff members. Provide training and/or direction as applicable to supporting government employees (e.g., hospital corpsmen, students, etc.).

* Attend and participate in various meetings as directed.

* Perform timely, accurate, and concise documentation of patient care.

* Operate and manipulate automated systems such as CHCS, AHLTA, ADS, Essentris, and Clinical Information System (CIS), participate in clinical staff Performance Improvement (PI) and Risk Management (RM) functions, as prescribed by the Commander. Maintain DOD email account as directed. The HCW shall be responsive to all email and voicemail communications.

* Exercise appropriate delegation of tasks and duties in the direction and coordination of health care team members, patient care, and clinic activities.

* Maintain documentation of all treatment provided in accordance with clinic directives, and prepare such records and reports as may be required. All records and reports must be legible. Abbreviations must be only those listed in local instructions.

QUALIFICATIONS:

* Education: Degree: Certificate. Graduate from an accredited community college, junior college, college, university or vocational nursing program approved by the appropriate State agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLNAC) or the Commission on Collegiate Nursing Education (CCNE).

* Licensure: Current, full, active and unrestricted license as a practical or vocational nurse.

* Experience: Possess a minimum of 1 year of full-time clinical experience within the last 2 years as a LPN/LVN.

* Certification: Possess and maintain a current American Heart Association BLS Healthcare Provider certification.

* Must be a U.S citizen and have the ability to pass a background check/security clearance.

STAFFING & SCHEDULING:

* Duty Hours: Each LPN shall provide services Monday through Friday for an eight and one-half hour shift between the hours of 0700 to 1730. The LPNs shift shall include an uncompensated one-half hour meal break. The LPN shall not exceed 80 hours of service in a two-week pay period.

* Place of Performance: 1) The work to be performed will be in support of the Naval Medical Center Camp Lejeune (NMCCL) Navy Comprehensive Pain Management Program (NCPMP). The primary site of service shall be Pain Management Clinic, NHCL. The LPN may be assigned to other clinic locations within the primary assigned Military Treatment Facilitys (MTFs) local commuting area. 2) The LPN may also provide scheduled tele-health services, in accordance with BUMEDINST 6300.20, to other providers and patients that present at Naval MTF or clinic locations.
