This position is responsible for effectively enhancing the drug therapy outcomes of patients at the Medical Center. This is accomplished through continual assessment of the appropriateness, safety, efficacy and outcomes of pharmacotherapy medication through direct patient care, education to health system personnel and the provision of pharmacotherapy-related outcomes research and performance improvement initiatives.

- Pharm D

- 3 years of experience

- NYS licensure

- Completion of a PGY2 Pharmacy Residency preferred OR completion of a PGY1 residency with a minimum 3 years experience in the area

of specialty

- Eligible for Board Certification in Pharmacotherapy or similar certification

- Eligible to be credentialed as an Allied Health Professional
