JOB DESCRIPTION: In accordance with organization standards and regulatory requirements, updates, edits, verifies, distributes, retrieves, and maintains computerized patient charts. May coordinate birth certificate certification. Analyzes medical records for deficiencies to ensure standards from various government and accrediting agencies and hospital are met. Responsible for accuracy, compilation and distribution of the daily census and creating monthly reports. Sorts and routes medical records by re-analyzing charts to appropriate location. Provides notification to physicians and assistance relative to physicians completion of medical records. Provides clerical and office support as needed; provides knowledge and direction on resources within the department as requested. Works with manager to formulate plan for professional development. Attends educational in-services as appropriate. Assembles, reviews, and verifies patient information in medical record folders and charts to conform to regulatory agencies and hospital documentation requirements; matches patient number and name to appropriate admission. Reviews, verifies, and updates admission and discharge lists to complete patient registration, including coding and recording of discharge date. Maintains statistical listings. Enters and updates data pertinent to physicians' record completion into master patient index file and chart tracking systems. Verifies that only one record exists for each patient. Reports discrepancies to supervisor. Regularly collects, organizes and compiles data and provides appropriate reports to managers. Retrieves charts for physicians; completes charts and delinquent records; updates databases; updates incomplete slip; and returns charts to the appropriate location in accordance with established procedures. Mails notices to physicians regarding incomplete records. Performs other related duties as required. MINIMUM QUALIFICATIONS: A high school diploma or equivalent. Two years experience in a medical record department or related health care experience. PHYSICAL REQUIREMENTS (MediumMax 25lbs): up to 25 lbs, 0-33% of the work day (occasionally); 11-25 lbs, 34-66% of the workday (frequently); 01-10 lbs, 67-100% of the workday (constantly); Lifting 25 lbs max; Carrying of objects up to 25 lbs; Occasional to frequent standing & walking, Occasional sitting, Close eye work (computers, typing, reading, writing), Physical demands may vary depending on assigned work area and work tasks. ENVIRONMENTAL FACTORS: Factors affecting environment conditions may vary depending on the assigned work area and tasks. Environmental exposures include, but are not limited to: Blood-borne pathogen exposure Bio-hazardous waste Chemicals/gases/fumes/vapors Communicable diseases Electrical shock , Floor Surfaces, Hot/Cold Temperatures, Indoor/Outdoor conditions, Latex, Lighting, Patient care/handling injuries, Radiation , Shift work, Travel may be required. Use of personal protective equipment, including respirators, environmental conditions may vary depending on assigned work area and work tasks.
