Acts is currently seeking a Social Worker/Bereavement Coordinator in our Hospice department.
The Hospice LSW develops, provides, documents and evaluates individualized social work assessment, counseling, and casework services to hospice participants and families as directed by the plan of care. The LSW counsels hospice participants and families on issues related to self-determined life closure, safe and comfortable dying, and effective grieving, in collaboration with the Interdisciplinary Team (IDT).
The critical skills and knowledge required to successfully perform the functions of the position are normally obtained through the completion of a graduate degree in social work with licensure in the state of Pennsylvania. Demonstrated competence in assessment, counseling, advocacy, resource management, and developmental and cultural diversity related to the end of life is necessary. Minimum of two years experience in hospice or a transferable, clinical venue of social work practice such as oncology, geriatrics, or community health, is required. On the job hospice training is required to learn hospice computer programs and agency policy and procedures.
