Job Description:
Perform the day to day operations of the intake coordinator for the home care agency, while adhering to all policies and procedure

Responsibilities Include:

· Arrange and coordinate all aspects of new patient’s intake into home health care. Staff qualified clinicians based on patients’ needs.

· Verify insurance eligibility and benefits of commercial insurance, Medicare, and Medicaid. Obtain authorization and to ensure services provided will be covered.

· Serve as a liaison/coordinator between clinical staff, referral sources, insurance companies, medical care partners, doctors, patients and family members.

· Establishes and maintains positive working relationships with current and potential referral sources.

· Manage and oversee Medical Records duties including MD order faxing and tracking and ensuring compliance with timeless per company policy.

· Complete consultations to educate new patients on admissions process.

· Communicate and coordinate patient care with patients, families, referral sources, hospital discharge planners, and clinical staff.

· Clerical duties such as copy/fax/file/order supplies as needed.

· Track received, accepted, and denied referrals for reporting purposes.

· Comply and enforce company policy and HIPPA.


· Education and Experience:

· LPN or any health care related degree or healthcare related an experience, Home Health Agency preferred.

· Must have excellent communication and customer service skills.

· Must be detailed-oriented and flexible to changes

· Strong interpersonal skills

· Must work well independently, and in a team environment

· Demonstrates good communication, negotiation, and public relations skills

· Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities

· Able to multi-task under time pressure
